Day to day Maintenance of E.I and Providing Lift Attendent for TCCC Building of Murshidabad medical college and Hospital including compound lights and TCCC Sub station for the period of 01 year (Electrical work) - Lift Attendent.

Charges for providing 01 no semi skilled lift attendant for 3(Three) No Lift (3 shift, i.e 8AM to 4 PM, 4 PM to 12 AM & 12 AM to 8 AM) as per given "Terms & Conditions" at TCCC Building, Murshidabad Medical College & Hospital, Murshidabad( Charges for overhead, Supervision administrative cost, bonus,ESI &PF etc).

N.B: Rate inclusive of all Taxes

TERMS & CONDITIONS

1. The contractor has to depute 02 No. Lift Operator (Semi-Skilled ) cum attendant in 01 Shift for 2 Nos. Lifts for 8 hours from 9:00 a.m. to 5:00 p.m. The contractor has to provide suitable substitutes for those on leave and as & when required as per requirement of the authority & timing of functioning of the authority throughout the year having a sound knowledge for operation functioning of the list satisfactory with Valid Lift Attendant’s Authorisation duly issued by the Licenceing Board, Government of West Bengal.

2. The Agency will have to submit the list of working Lift operator with reliever /substitutes to be engaged for the ensuring proper functioning /operation of lift installation mentioning their name & contact number No. category along with valid Authorisation Letter duly issued by the Licensing Board & identity proof and all these information with supporting documents are to be submitted in written to the concerned Junior Engineer (Electrical), Assistant Engineer (Electrical) & Executive Engineer (Electrical) prior to take up the work. This should be treated as mandatory.

3. The Lift Operator/Attendant should be dutiful and polite to all he has to keep watch on the lift for any difficulties.

4. The report is to be submitted periodically about the condition of the lift & all of its accessories with safety devices to the concerned Assistant Engineer (Electrical) & Junior Engineer (Electrical) with copy to the Executive Engineer (Electrical).

5. The Lift enclosure should be kept clean regularly by the Lift Operator

6. Penalty maybe imposed for non-operation of the lift in time as well as for absence of the operational staff the decision of the Executive Engineer (Electrical) will be final in this respect.

7. The agency should be responsible for any damage to the lift due to mishandling/ mal-operation done by his staff and shall be liable for the compensation for the same which would be recovered from the bill as per assessment & direction of E.I.C and the accused staff should be replaced immediately

8. The agency shall maintain one attendance register at site to be signed regularly by the staff on duty and the same should be authenticated verified by the concern Junior Engineer (Electrical) and Assistant Engineer (Electrical) the copy of this Attendance Registered duly counter signed by the concerned assistant engineer should be submitted along with the wheel

9. Any shutdown or fault should be brought to the notice of engineer in charge immediately in writing and over telephone for father course of action.

10. The engaged lift operator should be cautious so that untoward incidence, trapping of human being in the lift cage etc in respect of any malfunctioning of lift operation or other reasons should be attended immediately and the same should be set right as quick as possible to avoid any possible injury/ loss two human being.

11. The department would not be held responsible for any accident of the lift operator on duty for any reason the contractor or their lift operator /attendant would not claim any compensation in any shape from this department for such accident

12. The contractor has to engage supervisor once in a fortnight and as and when called for to supervise of all types of proper functioning/ operation of lift installation in this contract

13. Down time maybe allowed to bear minimum to attend false and repairing of any major /minor parts if urgently required in respect of restoration of lift in operating condition the detailed information in this regard i.e any problems and and the repairing maintenance jobs to be undertaken should be furnished to the undersigned with copy to the concerned Assistant Engineer (electrical )and Junior Engineer (electrical) well in advance however any negligence on the part of the agency is subject to u unilateral penalty if it detected on the spot or there after even after payment

14. The contractor shall be responsible to co-ordinate and lies with local authorities

15. The contractor will provide the workers with necessary testing and safety equipment and also follow all the safety instructions strictly.

16. Concerned Assistant Engineer reserves the right to terminate the contract against 1 month notice for satisfactory performance or other administrative reasons

17. The contractors will be held responsible for any damage of the equipments due to improper operation or negligence or lapses on his part

18. The agency should have a close to watch on the attendance of the lift operator as stated above in case of absence of any working staff the agencies shall have to arrange for suitable alternative within an hour failing which the charges for that day will be deducted from the bill of the agency on pro rata basis simultaneously the penalty may also be deducted from the bill for being failure to engage the lift operator on those days and the quantum of penalty would be decided by EIC depending on the gravity of situation the Assistant Engineer Berhampore Electrical Subdivision PWDte reserves the right to terminate the contract in case of absence of working staffs as stated above or more than three consecutive days without showing further reasons.

19. The agency has to provide one dedicated cell phone for single point contract regarding any preventive maintenance /breakdown issues however no claim of payment against any telephone bill will be entertained by these department

20. The behavior of working staffs of the entrusted agency should be polite in nature and may be withdraw from site in case of any adverse is reported however in normal cases frequent changes of working staffs will not be allowed without period official intimation to the concerned Assistant Engineer and Junior Engineer.

21. The philosophy of public interest would be the MOTTO of this all inclusive maintenance any deviation may be viewed or otherwise.

22. Any inconveniences caused so far as performance of lift installation due to negligence in the part of the agency if detected will be liable to penalty Quantum of penalty would be decided by EIC depending on the gravity of situation

23. The contractor must have their office and telephone contact with mobile number in Berhampore and the same should be intimated to this department in writing. The telephone call should be responded without delay

24. In case of any staff or any of the representatives of the interested agency are found to be engaged in any illegal activities that responsibility for the same should be borne by the agency

25. The agencies should be responsible for payment of minimum wages to the engaged workers as per relevant circular of Labour Department Government of West Bengal in vogue and non compliance of the same may lead to termination of the contract (minimum wages means daily rate as per recent circular of Labour Department Government of West Bengal corresponding to category of Labour X26 days)

26. The agency should submit PF and ESIs challan and payment acknowledgement document of the respective workers during submission of Bill. Bills should be submitted quarterly with the certification of concerned engineers on satisfactory completion (keeping at least one spare log book at site office) Duly supported with necessary checklist, lock sheets to the office of the Assistant Engineer Berhampore Electrical Sub division PWDte through proper channel payment will be made as per norms and availability of fund against placement of bills supported by log book duty verified by constant Junior Engineer/ Assistant Engineer Electrical

27. If any kind of incapability and negligence are noticed in observing all the same terms and conditions against the acceptance /agreement then necessary deduction in payment will be made in pro rata basis as decided by the Engineer in Charge in that case the above said contract maybe terminated without issuing any notice and stringent and punishable action maybe taken against the company
