Comprehensive annual maintenance contract for solar clocks - 4 Nos. installed at tower clock area of SJICR, Mysuru

Experience in executing similar work supported by documentary evidence to be enclosed (for last 5 years)

The bidder should have experience in annual maintenance contract of Solar clocks - installed at any hospital having 350 beds in Karnataka and their satisfactory experience certificates if any.

The agency should produce satisfactory experience certificates in the similar field at least from 5 clients in Karnataka. and also submit the documentary proof of hospital experience

Solvency from Nationalized/scheduled Bank for Rs. 2.00 lakh to be provided.

Income Tax returns filed for the assessment year 2024-25 & 2025-26

The agency should produce Balance Sheet for the last two years i.e 2023-24 & 2024-25.

Annual turnover for last 5 years certified by Chartered Accountant. (achieved in at least Rs.6,12,000-00 each year for the last two financial years )

Scanned copy of DD for Rs.2000-00 towards tender from charges

Names and Address of Organization

Whether proprietary / partnership/ Company/ Pvt. Ltd. (supported by documents).

Number of Technical Personnel and their duties.

The agency must and should enclosed copy of Registration certificate of PF, ESI, and labour license. Enclose (ESI &PF 6 months paid challan)

Value of works executed since last 3 years.

GST registration copy and returns filed for last six months.

Site Inspection counter signed by department head mandatory

The bidder company/agency should never have been black listed by any government organization. Affidavit in this regards to be submitted as in annexure iii.
