Users Certificate/s for Maintenance of similar minimum 300 KLD Capacity Sewage Treatment Plant, issued by Government/ Private Sector / hospitals / Government under takings/ Institutions in Karnataka for the last one year.

PAN Card, along with IT returns to be up loaded to the e-portal for minimum 03 years (2023-24, 2024-25 & 2025-26)

GST Reg Certificate and GST paid receipts / returns June -2025 to May - 2026 to be uploaded

EPF Registration Certificate, along with challans for having paid PF Contribution from June -2025 to May-2026

Annual Turnover statement for the years 2023-24, 2024-25, 2025-26 duly certified by Charted Accountant. The turnover shall not be less than Rupees Twenty Lakhs per year .

Tenderers Name, Address, Phone No. STD/ Mobile, e-mail ID details on the company letter head

ESI Registration Certificate, along with challans for having paid ESI Contribution from June -2025 to May-2026

If, the tenderer is a firm /Institution having executed the project of Erection commissioning & Maintenance of similar/ higher capacity Combined ETP/STP Plants, he shall furnish documents issued by the concerned Institution without fail, failing which the tender will be rejected. Registration Certificate as Authorised Service Provider if Tenderer is a service provider, issue by concerned Authority shall be uploaded.

Self-declaration on Rs.100/- Stamp Paper duly attested by Notary stating that the tendering firm is not blacklisted by any Government/Quasi Government Organizations/Institutions further stating that the documents submitted in the Tender are not false / erroneous. (Annexure-1)

Team member details and relevant information related to O & M of 300 KLD LETP/Sewage Treatment Plant/WTP.

Service Centre/s in Karnataka along with certificate of registration & Technical Staff available

If, the Tenderer is a Service provider, he shall enclose the KSPCB Authorized / empanelled laboratory certificate from KSPCB or Tie-up Letter from any of the KSPCB Empaneled Laboratory

ISO 9001:2015 Certificate of the Tenderer firm/Institution.
