Providing 2 No of computer operators to Revenue Section and Election Section service to CMC Chitradurga

The company/firm should have been in the business of providing manpower services for last 5 financial years.,( 2021-22, 2022-23,2023-24,2024-25 and 2025-26) and should have provided satisfactorily minimum 2 manpower service per year in a single order to any Government offices/Government undertakings. Certificates issued by the officer not below the rank of EE or Equivalent.

GOVERNMENT OF KARNATAKA DEPARTMENT OF LABOUR. REGISTRATION CERTIFICATE OF ESTABLISHMENT FORM-C (See Rule 4) The Agency should have valid registration certificate of establishment issued under Karnataka shops and commercial establishment act of 1961, according to government of Karnataka, department of labour and should have registered with minimum 2 labours engaged should be uploaded.

Karnataka Labour welfare fund (1965 Act) remitted receipts for the past five years 2021,2022,2023,2024 and 2025 with minimum 2 persons.

EPF REMITTANCE STATEMENT : The agency must have valid EPF registration certificate and EPF remittance statement minimum for Rs.1.44 lakhs (25% estimated cost)per Annum for having remitted for the past five financial years,2021-22,2022-23,2023-24, 2024-25 and 2025-26 duly certified by the office of the regional provident fund commissioner or competent authority as per EPF Act should be uploaded in original.

ESI REGISTRATION CERTIFICATE:- The agency must have valid ESI registration certificate issued by competent authority and attested remitted certificate of Contributions made for the past five financial years 2021-22,2022-23,2023-24,2024-25 and 2025-26. For minimum Rs.0.23 Lakhs (4% estimated cost) per annum from the concern department.

ANNUAL TURNOVER CETIFICATE:- The agency should submit the annual turnover for the financial years,2021-22,2022-23 2023-24,2024-25 and 2025-26 for Rs.11.58 lakhs per annum. (for man power only) issued & certified by the chartered accountant along with UDIN number with Profit and loss sheets. (2% estimated cost)

Unconditional Bankers certificate that the bidder has liquid financial resources of not less than Rs.1.73 lakhs (Credit lines/Letters of Credit / Certificates from any Nationalized/Scheduled Banks) for meeting the fund requirement etc., (Usually 30% of the amount of contract).

PROFESSIONAL TAX REGISTRATION FORM No. 4 CERTIFICATE:- The Agency must have certificate of Enrolment issued under Karnataka tax on professions, traders, calling and employment act 1976 (Form-4) or its equivalent should be uploaded. The remitted receipts for the past financial years, 2021-22, 2022-23,2023-24,2024-25 and 2025-26 should be uploaded.

GST REGISTRATION REG 06 CERTIFICATE:- The agency should have GST REG-06 registration certificate and should be uploaded. GST Should be Registered before 5 financial years.

GST remittense certificate for last five financial years 2021-22, 2022-23,2023-24,2024-25 and 2025-26 duly certified by the GST Commissioner or competent authority should be uploaded.

Anti-collusion certificate as in Appendix G Self-declaration certificate regarding not black listed by any firm/ Government agencies in the format set out in Appendix G1
