Previous Experience in similar assignments of the Bidding organization Maximum of 30 marks Mandated Previous experience details to demonstrate in depth work experience on the use of participatory methodological approaches for community engagement and joint priority setting, and handling of Medical Health related awareness programmes, with history of collaborative partnerships with any State Central Government Departments during the last three financial years (Financial Year 2022-23, 2023-24 and 2024-25). Appendix V must be uploaded along with valid documents. The applicant organization (s) must also provide proof of work, with documented experience in Karnataka or other States. This is to ensure the presence of diverse 30 understanding and knowledge of translatable best practices and learnings relevant to the present project context. For 1 Projects 10 marks for 2 Projects 20 marks for 3 and more Projects 30 marks Documents to be uploaded WORK DONE CERTIFICATES issued by Central State Government Departments for the last 3 (three) years.

The Bidder may be registered under section 12Aof the Income Tax Act, as a Non Profit Organization.(Maximum of 05 marks) Appendix I, II, III, IV must be uploaded as applicable to the bidding firm. (Mandated)

The bidder must be registered with the EPF authorities. EPF Registration Certificate and month wise EPF Payment Confirmation Challan from April-2024 to September-2025 must be uploaded. (Maximum of 05 marks)

The bidder must be registered with ESI authorities. ESI Registration Certificate and month wise ESI Payment Confirmation Challan from April-2024 to September 2025 must be uploaded. (Maximum of 05 marks)

Composition of Multidisciplinary team (Maximum 20 marks)(Mandated) (Appendix VI, VII, VIIA, VIIB)The applicant organization must have a multidisciplinary team with widespread experience of at least 20 years and available to devote significant time to fulfilling the requirements of this RFP.(As per tender Document)

Work Plan and Methodology for carrying out the operations/services for 2 years. (Mandated) The Bidder has to upload the detailed work plan and the methodology of carrying out the services operations as per the Scope of Work as detailed in the RFP. (Maximum of 20 marks)

Tenderer should upload the undertaking regarding Non Conviction Certificate as prescribed in Appendix-VIII. (Mandated)

Tenderer should upload the No Deviation Certificate as prescribed in Appendix-IX. (Mandated)

Tenderer should upload the Undertaking regarding Blacklisting / Non Debarment as prescribed in Appendix-X. (Mandated)

The bidder must have uploaded Income Tax Returns for the financial year 2022-23, 2023-24 and 2024-25. (Maximum of 05 marks) (Mandated)
