The bidder shall upload an affidavit that the bidder is solvent as per Annexure -A4.
The Bidder should be either : a. Apex dairy Co-Operative Societies approved by Government. b. Dairy Co-Operative Federations/Members of National Co-operative Dairy c. Federation of India Limited (NCDFI) and/or developed under the aegis of Operation Flood Programme, d. Bodies/agencies registered /certified by FSSAI for sale of milk and milk products. In case of firm registered under the Partnership Act 1932, please enclose details of partners alongwith certificate of registration, details of their business and partnership deed, etc. duly attested by Notary.
The Bidder should have a minimum of three (3) years of experience in sale of milk and milk products to be supported by turnover(Income tax return/Bank statement) in that business for the last three years duly certified by Chartered Accountant. The Bidder shall upload a copy of Articles of Association / Partnership Deed where catering / hospitality / food and beverage is covered under the Main Objects clause of the agreement

Annexure A/1 shall be downloaded, filled, signed and uploaded (Page no. 15 - 16 of the bid document)
Annexure A/2 shall be downloaded, filled, signed and uploaded with supporting documents (Page no. 17 - 18 of the bid document)
Annexure A/3 shall be submitted on non-judicial stamp paper of value Rs.200/- duly notarized. Copy should be uploaded and original shall be submitted within 14 days from bid opening (Page no. 19 of the bid document)
Annexure A/4 shall be downloaded, filled, signed and uploaded (Page no. 20 of the bid document) Copy should be uploaded and original shall be submitted within 14 days from bid opening
Annexure A/5 shall be downloaded, filled, signed and uploaded (Page no. 21 of the bid document)
Audited Balance Sheet and Profit and Loss accounts or Income Tax returns of previous three financial years, viz, 2022-23, 2023- 24 and 2024-25 duly certified by Chartered Accountant shall be submitted.
Copy of Govt approved identity proof such as Aadhar Card
Copy of FSSAI certificate
Power of Attorney for signing the bid (applicable to all bidders except individual bidders, to be executed on non-judicial stamp paper of value Rs.200/-) as per format at Annexure A/6 to be uploaded alongwith other requisite credential/documents and original shall be submitted within 14 days from bid opening.
Copy of the GSTIN registration with GSTIN number shall be uploaded
Copy of the PAN card with PAN number shall be uploaded
Copy of the PF registration certificate with PF registration number, if available, shall be uploaded
Attested copy of the Articles of Memorandum or attested copy of the Partnership deed shall be uploaded, if applicable
The bid document should be digitally signed and uploaded
