Turnover of the Bidder for the preceding three years i.e. (2022-23, 2023- 24 & 2024-25) from Sale of Milk and Milk products (Please enclose a statement duly certified by a Chartered Accountant as per Annexure 5)

Income Tax Returns / Bank statements of the bidder for the preceding three years viz. (2022-23, 2023-24 & 2024-25) (Please enclose a statement duly certified by a Chartered Accountant).

Minimum three years of experience in sale of milk and milk products to be supported by turnover (Income Tax Return/Bank Statement) in that business for the last three years duly certified by Charted Accountant.

The Bidder shall submit a copy of Articles of Association / Partnership Deed where catering / hospitality / food and beverage is covered under the Main Objects clause of the agreement.

Specify the Legal Status of the Bidder: 1)Apex dairy Co-Operative Societies approved by Government, 2)Dairy Co-Operative Federations/ Members of National Co-Operative Dairy Federation of India Limited (NCDFI) and / or developed under the aegis of operation Flood programme, 3)Bodies/Agencies registered/certified by FSSAI for sale of milk and milk products.

In case of Firm, registered under the Partnership Act., 1932, Please enclose details of partners along with certificate of registration, details of their business and partnership deed etc. duly attested by Notary.

Scope of activity along with certified copies of certificates with whom the bidder has been registered. (Please enclose the details of Registered Office along with details of state wise/City wise units).

PAN No. of the bidder (Please enclose the self-attested photocopy of PAN Card issued by Income Tax Department).

Aadhaar of the authorized signatory.

GSTIN Number of the bidder (Please enclose the self-attested photocopy of the GST registration certificate issued by relevant authority).

FSSAI Certificate No along with self-attested copy of certificate.

PF registration of the bidder if applicable as per Government of India EPF Act, amended from time to time. (Please enclose attested photo copy of certificate issued by Provident fund commissioner).

Covering Letter comprising the Bid (Annexure - A/1) duly filled and signed.

Mandatory Information for Eligibility (Annexure - A/2) duly filled and signed.

Affidavit on number of Milk Stalls held over Vadodara Division of Western Railway(Annexure - A/3) to be submitted on non-judicial stamp paper of Rs 100/- and duly attested by a notary public.

Declaration for solvency(Annexure-A/4) Affidavit Self- Declaration Form to be submitted on Non-Judicial Stamp Paper of Rs.100 and duly attested by a notary public.

Form Tech-1 (Annexure-5) Turnover of the bidder from Milk business in the preceding 3 financial years.
