The tenderer should attach the work experience and service satisfaction certificates obtained from Government/Semi-Government organizations regarding the experience of providing 379 staff for at least two years during the last 3 years during the years 2022-23, 2023-24 and 2024-25. (Form-7)

Submission of registration certificate under the E.S.I Act and Challan copy regarding E.S.I filled for the year 2024-25. (Form-3)

A registration certificate (minimum 300 employees) in Form C should be submitted regarding registration with the Labour Department of the Government of Karnataka and should be valid for the current year

Elected members, blacklisted organizations will not be allowed to participate in the tender. And a copy of an affidavit obtained from a notary on a stamped paper of Rs.200.00 stating that they are not included in the blacklist and that they are not an elected member. (The said affidavit will be obtained and submitted specifically for this tender invited on 28/04/2026)

A letter of permission from the Municipal Corporation/City Council/Town Panchayat/Municipalities and other competent authorities regarding the establishment of the tenderer/agency. It should be valid for the current year

The tenderer/agency should have an office locally. Necessary documents in this regard should be submitted. If there is no office, a letter of undertaking to open an office should be submitted if the tender is accepted

EMD amount and details of deposit (Details submitted under e-procurement)

The tenderers should be financially sound. In this regard, 30% of the tender amount should be submitted in the name of the Deputy Director, Social Welfare Department, Raichur District, along with a Bank Solvency Certificate from a Nationalized Bank

Submission of Professional Tax Department Registration Letter and Certificates of Professional Tax Payment for the Year 2024-25 (Form-1)

Registration under the G.S.T Act and G.S.T. Copies of G.S.T. Returns paid for the year 2024-25. (GSTR-3B)(Form-4)

Submit a copy of the PAN CARD in the name of the organization/individual.

Submit Income Tax Returns and Audit Report with details of Income Tax Payments for the financial years 2022-23, 2023-24 and 2024-25 along with Balance Sheet & Expenditure Schedules to the Income Tax Department. (Should be in the name of the organization/individual) 3CD & 3CB (Form-5)

The institute should have an annual turnover of Rs. 94851372/- in each of the last three years (2022-23, 2023-24 and 2024-25). A certificate in this regard from a Chartered Accountant (CA) should be submitted to the concerned authority with UDIN number. (Form-6)

Copy of license obtained from the Director General of Police (Internal Security).

ISO registration certificate must be submitted
